Graham House Community Transport Services is an accredited Transport Provider with Queensland Transport and offers a range of transport options for transport disadvantaged people of the South Burnett Region.

SBCITS (South Burnett Community Integrated Transport Service)

SBCITS is subsidized by Home and Community Care (HACC), a joint Commonwealth-State funded program and provides transport to frail elderly transport disadvantaged residents of the South Burnett.

Community Partnership Transport Service
CPTS provides non-acute, non emergency ‘out of area’ medical type transport for people who are transport disadvantaged and meet the eligibility criteria of the service.

A coordinated community transport scheme that increases access to local services and improves community networks and connections.

SBCITS (South Burnett Community Integrated Transport Service)


SBCITS aims to deliver to HACC clients, a coordinated community transport service in the South Burnett. SBCITS offers the South Burnett Community, coordinated, planned transport options to eligible clients who are transport disadvantaged.

 

The options in delivering this service mean you may be linked in with an existing service such as a bus route; or a volunteer driver may provide you with transport. Alternatively, you may share a ride with another person with a similar destination. If there is mainstream transport available to you, and you can use it, we will not provide a direct service in one of our vehicles.

Transport assistance is provided so that clients may get out of their house and do chores, for example, shopping, banking, visits to the library, local doctors and attend other activities or community centres.

Eligibility Criteria
Clients need to:

  • Be a resident of the South Burnett area; and

  • Be a frail older person who has difficulty with everyday tasks; or

  • Be a younger person with a moderate to severe disability;or

  • Be a carer of one of these people and

  • Be living at home; and

  • Be transport disadvantaged.

Community Partnership Transport Service

Community Partnership Transport Service (CPTS) provides non acute, non emergency medical type transport for people who have a short-term non acute medical condition or a frailty that prevents them from using conventional private or public transport and who are transport disadvantaged and meet the eligibility criteria of the service.

Client Eligibility

  • Be a frail aged or young person with a functional disability who cannot reasonably gain access to health services by either public or private means; and
     

  • Be able to provide the service with a medically authorised transport form signed by their health professional eg. Doctor or Registered Nurse stating that the transport service will be suitable for their needs.
     

Type of transport
 

CTPS can provide direct assistance to eligible clients for the following:
 

  • visits to medical specialists;

  • visits to hospitals prior to some procedures;

  • follow up visits to specialists;

  • transport for some medical treatments
     

Service Cost

CPTS is not a funded service and cost recovery for each trip is an important factor. Financially disadvantaged persons are encouraged to make prior arrangements with the CTPS staff to discuss payment options. Vehicle kindly funded by Jupiters Casino Community Benefit Fund and Tarong Energy

 

 



B4B Transport Solutions

Transport Solutions is a transport coordination project funded by the State under the Blue Print for the Bush Rural Access Initiative. The service was established in 2007 and aims to provide the South Burnett Community with coordinated, planned transport options to people who are transport disadvantaged. The service is available to assist people who may be having difficulty finding suitable local transport. Through this service transport disadvantaged people may be able to link into other support groups, public transport or community transport resources where appropriate and available.

The Transport Solutions target group is families and isolated people with a focus on activities such as business, medical and educational.

Client Eligibility

People requiring this service would be considered transport disadvantaged if they are without access to regular, affordable and appropriate transport to medical, business, educational and social engagements.
